Are you a DVA Card holder or eligible for assistance through Medicare?
Department of Veterans’ Affairs – Gold Card
If you are a veteran with a Repatriation Health Card – For All Conditions (Gold Card) or a veteran with a Repatriation Health Card – For Specific Conditions (White Card), you may be eligible to consult with an Accredited Exercise Physiologist (AEP) in a one on one or group setting though condition do apply.
Coordinated Fitness’s AEP’s can help you in the management of diabetes, cardiovascular disease, osteoporosis, arthritis, cancer, chronic muscular conditions and many other health conditions.
To consult with Coordinated Fitness for these services you must obtain a referral from your GP or Specialist. The referral form required is a D904 form or a normal referral letter with your health condition and DVA number.
All DVA clients are bulk billed so there are no out of pocket expenses to you.

Medicare – Enhanced Primary Care Programs
Under the Medicare – Enhanced Primary Care Program people with specific chronic health conditions maybe eligible to consult with an Accredited Exercise Physiologist whilst receiving a rebate of $50.95 from Medicare. For more information click on the link Enhanced Primary Care Programs
Under the Medicare Allied Health Group Services for Type II Diabetes clients with Type II diabetes can complete a 4 week group program with Coordinated Fitness. For further details on this program please click on the link Medicare Type II Diabetes Program
Your health assessment will include:
- Health Screen
- Postural Assessment
- Fitness Assessment
- Waist and Hip Measurements
- Skin Folds (if you want them done)
- Goal Setting
- Nutritional Assessment
- Lifestyle Assessment
The “Enhanced Primary Health Care Plan Scheme” delivered by Medicare enables people with complex medical conditions to receive rebates for their health service provided by an Accredited Exercise Physiologist (AEP).
Coordinated Fitness’s AEP’s can develop a management plan to assist people with chronic health conditions. Our goal is to design appropriate exercise and lifestyle plans to manage current chronic diseases of the patient and to assist in preventing the development of new chronic diseases.
Conditions Coordinated Fitness Exercise Physiologists assist with include

- Cardiac Conditions

- High Cholesterol

What do you need to do to determine if you are eligible?
If you have a chronic health condition ask your doctor if you meet the criteria set by Medicare Australia. If you do you will need to be referred by your GP with the relevant paper work for the Allied Health Service Provider. Eligible clients will be entitled to a maximum of five (5) allied health visits per calendar year, including Exercise Physiology services.
